School & Sport Physicals (Knox Community Health Center)

The Knox Community Health Center is offering school & sport physicals by appointment only on specific dates during the summer. Please call 309-344-2225 June: 8, 9, 27, 29 July: 20 & 21 August: 1, 3, 8, 10 $35 School Physical $15 Sport Physical immunization costs will vary. Please bring immunization records to your appointment! Individuals do not have to be patients in order to receive physicals.
